Just had a client pc on a small domain, 15 clients, to crash. So I built a new screaming machine w/Windows 7 Ultimate. The old pc ran XP Pro. I tried to name the new computer the same as the old one that's dead, but I get an error saying there is already one on the domain by that name. I think it has something to do with the DNS name (a completely uneducated guess on my part) but can't seem to figure it out. I'm very new to servers and domains.

Open Active Directory Users and Computers on your server. Look under the Computers folder unless you move your machines to their own OU. Find the old computer name and delete it. Then join the new PC to the domain.

Will this resolve the Exchange Server issue as well?

Sorry, hands can't keep up with brain. I added her pc to the domain, do I have to delete her old mailbox in Exchange then add a new one?

No, the mailbox is tied to the user account and not the computer account.
